A colleague mentioned in passing that our supervisor had made some changes to a piece of safety equipment. While I had no reason not to trust his work, I decided to check it out anyway.
Knowingly or unknowingly, he had disabled not one, not two, but all three safeties, either misinterpreted the results or skipped testing altogether, and put hundreds of millions of dollars worth of equipment and hundreds of lives at risk, including mine.
It took a long time but I stayed until it was safe. And then I changed career.
