Yes, the first issuance of a passport as an adult (over 16), or any passport issued to replace a lost/stolen passport requires a quick oath before the person who accepts the application. Something to the effect of the following:
"Do you swear or affirm that everything you've written in this application is true and complete to the best of your knowledge?"
