It's not the one thing, because I have a lot of stuff that I don't regret buying. But a couple of things that I use daily and seem to be very well-made and comfortable are:
Kensington Trackballs
If you work with a mouse all day every day, you can easily develop repetitive stress injuries. A trackball minimizes arm movement, and even finger movement if you turn the sensitivity way up. And Kensington gear is just really, really solid. All the buttons are reprogrammable and the ring lets you scroll. I have never known one to break.
Microsoft Natural 4000 keyboard
I know it looks really weird, but this split keyboard lets you keep your wrists straight while you type, rather than bending them to fit a straight keyboard. Once you get used to it (which doesn't take long), you won't want to go back to a regular one again.
